Offer: Credit, not to exceed $100,000, which Customer may elect to: (i) receive as a credit on an invoice; or, (ii) have deposited in Customers Fund account. For Customers who elect to receive the credit as an invoice credit, the credit will be applied:
in the sixth monthly period following execution of Customers written agreement with the Company for service under this promotion for a Customer who subscribes to service under a 1-year term of service,
in an amount which equals 50 percent of the credit amount in each of the sixth and eighteenth monthly periods following execution of Customers written agreement with the Company for service under this promotion for a Customer who subscribes to service under a 2-year term of service, and
in an amount which equals 33.33 percent of the credit amount in each of the sixth, eighteenth and thirtieth monthly periods following execution of Customers written agreement with the Company for service under this promotion for a Customer who subscribes to service under a term of service which equals or exceeds three years.
For Customers who elect to receive the credit as a Fund deposit, the credit will be applied within 30 days of the date of full execution of Customers written agreement for a new term of service under this promotion and will be equal to the following percentages of Customers annual volume commitment for Company service, based on Customers term of service.

Definitions: For purposes of this promotional offering only, Qualified Commercial Affinity Group is defined as a trade association representing business entities or individuals within an industry, professional or business classification, or a commercial organization with affiliated franchisees, independent agents, independent distributors, businesses in common or other multiple commercial representatives or a buying group not organized for the purpose of qualifying for the discounts provided herein, which agrees to meet a defined set of criteria within 6 months of undertaking to qualify and thereafter maintain them, and enters into an agreement with the Company for the marketing of the Company's services. The Group must achieve and maintain aggregate billings, exclusive of FUSF and CAC charges, Public Payphone and Restricted Line Surcharges, promotional credits, surcharges, and taxes.
Eligibility: Customer must:
enroll between June 1, 2003 and February 28, 2005;
be a member of a participating Qualified Commercial Affinity Group; and,
subscribe under a new term of service to audioconferencing, Cross Border Private Line Services, Frame Relay Services, International Private Line Service, Metro Private Line Access Service, Data Center Services, Internet Dedicated Services, Internet Dial Corporate, MCI Advantage Service, Voice Services, Company-provided exchange service, Company-provided ATM service, Company-provided DSL service, IP VPN Dedicated Services, IP VPN Remote Services CPE Based, IP VPN Remote Services Network Based, and/or Private IP Service.
Credit under this promotion may not be applied to taxes, charges for unauthorized calls, prior outstanding balances owed to the Company, termination charges, underutilization charges, or disputed charges.
If Customer terminates the term of service: (i) prior to the monthly period in which the credit is to be applied, Customer will not receive the credit; and/or, (ii) prior to expiration of the term of service, Customer may not apply credit remaining at the time of termination.
Customer may not receive (i) any discounts, or (ii) the benefits of: Checkbook Promotion 2004; Regional Checkbook Promotion 2004; or any Special Customer Arrangement (SCA) or Product Package, other than Product Package Guide Types 13, 14, 15, 16, 18, 19, 20 and 21.
